Reasons for Implant Failure and Ways to Avoid Them

Dental implants are titanium posts, placed in the jaw bone, help accommodate replacement teeth permanently to give a pleasing effect aesthetically and functionally.

There are two types of Implants

Reasons for Implant Failure and Ways to Avoid Them

1) Conventional Two-piece implant: As the name suggests consist of two components, the root and the tooth component which are connected using a screw.
2) The Basal implants, single piece implants: Here, the tooth and the root component are fused to present a unified structure.

Dental implantation is an innovation in the field of dentistry, which helps regain a person’s self-esteem and confidence lost due to missing teeth and fitting removable dentures.

Even though there are many benefits of dental implants, there is a risk that they might fail and the reason is multifactorial, Oral manifestations like constant pain and mobility even weeks after implant placement indicate an issue with the treatment. However, what happens if and when they fail? Can we fix it? Can you get another implant? So, what can you do if one or more of the symptoms appear? What treatment options are there?

Conventional Dental Implant

Case selection plays a vital role in the success of Conventional Implants. These implants require an excellent bone for immediate loading, and if the bone quality and quantity is not proper, they need bone augmentation and sinus lift procedures to create a favourable site for implantation, which can be done once the surgical site heals, and the teeth can be loaded accordingly( delayed loading)

Reasons For conventional Dental Implant Failure

There are many reasons why conventional dental implants can fail, and these causes can lead to early or long-term failure.

Early Dental Implant Failure

Initial dental implant failure occurs within three to four months of the procedure during the process of integration of the jaw bone with the implant.

Here are some common reasons for early dental implant failure:

Poor Bone Quality/Quantity:

If the bone volume is not proper, and the dentist plans for an immediate loading without executing procedures like bone grafting and sinus lift procedures, the implants tend to fail.

Overheating:

Inadequate irrigation and overcompression of the bone during implantation might lead to necrosis of the bone resulting in implant failure.

Lack Of Primary Stability:

It is nothing but the mechanical engagement of the surrounding bone with the implant, bone quality/quantity and the technique involved in placing the implant influences this, if primary stability is not attained during implant placement then the implant may not osseointegrate with the bone.

Incorrect Immediate Load Indication:

It is very crucial to follow strict guidelines and a definite surgical approach while selecting a case for immediate loading using conventional implants

Medications

Some drugs can prevent the success of dental implants. That is why it is essential to tell your dentist about the medicines that you are using.

Infections

Reasons for Implant Failure and Ways to Avoid Them

Infection can destroy the chance of implants for successful integration. One common infection with dental implants is peri-implantitis, a destructive inflammatory process affecting soft and hard tissues surrounding dental implants.

This infection can occur during or after surgery; In most cases of peri-implantitis, dental implants can be replaced after a short period. Patients with autoimmune diseases, smokers, or patients with poor oral hygiene are more likely to develop this infection.

Overloading:

In a typical conventional implantation procedure, the dentist does this in two steps: placing the implant and placing the abutment and crown. The time between these two stages allows the implant to osseointegrate into the jaw bone before the abutment and crown are placed.

To shorten treatment time, sometimes the dentist will do, what is called “immediate loading procedure.” However, this procedure can cause excessive damage and implant failure caused by excessive force or pressure on the crown. This can be a problem if the patient has poor bone density, and proper case selection is not done.

Nerve/Tissue Damage

If the nerve or tissue around the implant is damaged, the patient may experience constant pain or numbness after the procedure. The sensation can spread from the area to the cheeks, tongue, chin, and lips.
Nerve damage is caused due to an inexperienced dentist, placing an implant too close to a nerve.

Late Or Long-Term Failure

If a dental implant fails one to ten years after surgery, it is considered a long-term failure. Several factors can contribute to dental implant failure.

Doctors Experience

Reasons for Implant Failure and Ways to Avoid Them

The qualification and the experience of the performing dentist plays a crucial role in the success of the implant as it is a specialised treatment.

Smoking

Smokers face a higher risk of failed conventional dental implants. So smoking is a risk factor for failed conventional dental implants. Smoking is contraindicated in conventional two-piece Implantation.

Bruxism

These are caused due to many reasons, such as Stress, irregular teeth, or missing teeth. This grinding or clenching adds additional pressure to the teeth and causes wear or damage.

Head And Neck Radiation

If a patient is suffering from cancer-related to mouth and undergoing radiation therapy, then this increases the risk of implant failure.

Mechanical Damages

Some accidents can lead to dental implant failure.

Symptoms Of A Dental Implant Failure

  • Some of the dental implant failure Symptoms:
  • swelling or inflammation
  • Severe pain or discomfort in the jaw
  • Gum recession around the dental implant
  • Feeling difficulty chewing and biting
  • A loose or shifting implant

What You Can Do for Implant Failure :

How do you care about potential failure depending on the reason? However, for whatever reason, the first thing you need to do is contact your dentist. If the implant area is infected, try rinsing with an antiseptic mouthwash regularly, until you see a dentist.

Even in this case, good oral hygiene is very important. The faster you see infection and handle it, the better.

Basal Implants:

Basal implants are single piece implants which can be placed in any patient above the age of 18 years irrespective of their health status and bone condition as they are placed in the basal bone which is highly mineralised and is present in all, regardless of the amount of infection present surrounding the teeth.

Indications:

1)Partial or complete loss of teeth
2)people vexed up with conventional dentures
3)patients who cannot wait for months together and want immediate results ( permanent fixed teeth in 3 days)
4)Also indicated in smokers
5)Basal implants come with a tagline,” Implants for all”, which means that they can be placed in any bone regardless of the systemic health condition and the habits of the patients

Contraindications:

  • Patients on IV bisphosphonates
  • Patients undergoing Radiation therapy( Only during the therapy, six months after the therapy the patient can undergo Basal implants)

Reasons For Failures

Reasons for Implant Failure and Ways to Avoid Them

1)Performing doctors surgical hand and experience holds utmost importance as they are immediate loading. Implants tend to become loose in the long run if they are not properly placed.
2) Implants fail if the remnant infection is not cleared properly after tooth extraction
3)Chances of failure, if the implants are overloaded, that is, if a sufficient number of implants are not placed.
4)If implants are placed in contraindicated patients like patients on IV Bisphosphonates and patients undergoing Radiation therapy.

Conclusion:

Implants are the biggest breakthrough in the field of dentistry with about 97% success rate, so failure is a rare phenomenon. Also, you now know that the best way to prevent and avoid implant failure is approaching a proper Implantologist and proper case selection. Conventional implant treatment is indicated in patients with good bone volume and basal implants can be placed even in patients with inadequate bone volume and smokers without undergoing any bone grafts as they utilise the basal bone which is present in all and conventional implants can be placed in low bone volume cases after sinus lift and bone grafting procedures.
